All forms are provided as reference materials for students currently enrolled in a To Write Art program and may be downloaded as needed throughout the year. No forms require completion prior to enrolment.

Pre-topic questionnaire capturing students' initial thoughts, expectations, and existing knowledge before beginning each topic.

Post-topic reflection documenting learning outcomes, perspective shifts, and how the topic influenced their artistic thinking.

Consent and certification form for students submitting artwork for website/Instagram sharing, including watermark preferences and copyright acknowledgements.

Request form for removing previously submitted artwork from To Write Art platforms, with 48-hour processing timeline.

Parent/guardian authorisation for students under legal age to participate and optionally submit artwork for online sharing.
